to submit an application form
To submit an application for Paratransit service, complete
the official forms (parts 1, 2 and 3) for the category of
your impairment:
- Motor or organic
- Intellectual
- Visual
- Behavioural
The forms are available in pdf format at the bottom of this
page.
Instructions for applicants
1. Complete Part 1, General Information.
This part can be completed by the applicant.
2. Have Part 2, Diagnosis,
completed.
- This part is for the diagnosis of each impairment. Applicants
must have their health professional complete this section
of the form on the diagnosis of their impairment. It can
be completed by:
- A family doctor or specialist, such as a cardiologist,
pneumologist or neurologist for a motor or organic impairment;
- A family doctor, psychologist or psychiatrist for
a behavioural or intellectual impairment;
- An optometrist or ophthalmologist for a visual impairment.
3. Have Part 3, Functional Assessment,
completed.
- This part assesses the limitations of the impairment
with respect to the applicant's ability to use regular public
transit.
- Unless it is obvious that the applicant always uses a
wheelchair, this part must be completed by:
- An occupational therapist, physiotherapist or rehabilitation
therapist for a physical or motor impairment;
- A psychologist or psychiatrist for a behavioural
impairment;
- An educator or social worker for an intellectual
impairment;
- An orientation and mobility instructor for a visual
impairment.

5. The Eligibility Committee, which meets once a month, reviews
the application according to the criteria set out in the Paratransit
Eligibility Policy from the Quebec Ministry of Transport (MTQ),
and informs the applicant of the decision in writing within
a few days.

The
key role of the professional from the Réseau de la
Santé et des Services Sociaux
The role of the professional is very important. The forms
certifying an impairment that are completed by the professional
(Part 2 and 3)must contain enough information to answer the
following questions:
- Is the impairment significant and permanent?
- Is the impairment severe enough with respect to mobility
to justify the use of the paratransit service?
- Would the person be capable of using the regular public
transit service after a rehabilitation period or after taking
a training or familiarization program?
It is important for the forms to be completed correctly,
otherwise the Eligibility Committee cannot reach a decision.
A new form will then have to be sent to the applicant and
processing of the application will be delayed.
