FIDÉLITÉ Program - Program Conditions

FIDÉLITÉ Program

- A duly completed and signed debit authorization and a cancelled cheque must be sent to the Finance Department at the Société de transport de l'Outaouais.

- A new personalized smart card will be issued following the application for enrolment in the Fidélité Program.

- The card is programmed for use starting on the 1st day of the month following the first debit and will remain active as long as the plan is active.

- The debits to the bank account will be every month on the 25th day or next office day, if a holiday. Each debit equals 90% of the full fare in effect for a monthly pass in the same category.

- You must enrol prior to the 15th day of a given month to have a pass issued on the first day of the following month.

RENEWAL AND CANCELLATION :

- The FIDÉLITÉ Program annual pass with pre-authorized debit payment is renewed automatically yearly unless written notification is submitted to the Finance Department at the STO at least 30 days prior to expiry. When applicable, the smart card will be cancelled on the day following the plan expiry date.

- The smart card must be returned to the STO either by mail (111 Jean Proulx, Gatineau, J8Z 1T4) or at one of the STO customer service counters within 30 days of the plan expiry date. Failing this, an additional debit of $20.00 will be processed on the holder's account.

- The STO reserves the right to cancel the smart card should any fraud be determined during processing of the file (false statement, falsified documents, etc) or in the case of a default in payment.

- Should a debit be rejected or the plan cancelled before expiry, you will be held liable for the discount applied on your early plan for each month of use of your smart card (every month that the card has been used) in addition to the amount of $20.00 if the smart card purchased under the FIDÉLITÉ Program is not returned to the STO within the 30 days following notification from the STO. The smart card purchased under the FIDÉLITÉ Program will be cancelled at the end of the current month.

- NSF charges (except for technical incidents beyond the control of the payer) will be the responsibility of the payer.

- The STO reserves the right to deny access to the plan for a holder whose smart card has been rightfully cancelled.

- The plan may be cancelled at the request of the payer, without any fees owing to the Société de transport de l'Outaouais, when the holder is unable to use the smart card for a period exceeding three months. Only the following situations are applicable:

. Holder's death, a certificate must be provided.
. Extended illness or accident, a certificate must be provided.
. Unexpected move outside the Outaouais area; proof of the new address must be provided.

In all cases, the STO's approval is required for the cancellation to become effective.

- The STO may terminate this plan or modify the conditions by giving 30 days' written notice. When applicable, the holder will not be held liable for the savings to date, but may be required to return the smart card to the STO within 30 days following the enforcement of the notice, failing which an additional debit of $20.00 will be processed on the account.


CHANGE OF FINANCIAL INSTITUTION:

- The STO must be informed of any changes in financial institution.
You must submit a new duly completed and signed pre-authorized debit application accompanied by a cancelled cheque. The STO must receive the new authorization before the 15th day of the month in order for processing to occur in time for the next debit period. Consequently, you must take the necessary measures to ensure that there is no disruption in the monthly payments.

MOVING:

- If you move, you must inform the STO of your change in address.


MISCELLANEOUS:

. Holders of a «Programme Fidélité» smart card agree to comply with STO regulations, in particular when boarding buses. You must pass the smart card in front of the reader at the front of the bus, wait for the green light and than show show the driver the side of the card with your photo.

. Lost smart cards can be reissued for a fee of $10.00. Defective cards will be replaced at no charge.

. Contrary to the monthly Passe-partout PLUS pass, a smart card issued under the FIDÉLITÉ Program - Annual pass with pre-authorized debit payment, does not require a proof of purchase (receipt) to use the OC Transpo system. Except for this difference, the smart «Programme Fidélité» card is subject to the same rules as other passes regarding the supplements required on the OC Transpo express routes.
