TERMS AND CONDITIONS |
|
To enrol in the “FIDÉLITÉ Program”,
applicants must complete the enrolment form, sign the
personal PAD (pre-authorized debit) authorization and
submit everything, accompanied by a voided cheque, to
the Finance Department at the Société
de transport de l'Outaouais.

Enrolment
must be prior to the 15th day of a given month to
start the following month.

A
new personalized Passe-Partout Plus pass will be
issued upon receipt of the application for enrolment
in the FIDÉLITÉ Program.

The
Passe-Partout Plus pass is programmed for use starting
on the 1st day of the month following the first
debit.

Bank
account debits are taken on the 25th day of every
month or the next business day, if this is a holiday.

Each
debit equals 90% of the full fare for a monthly
pass of the same type.

FIDÉLITÉ ENROLMENT RENEWAL |
|
Enrolment in the FIDÉLITÉ Program is annual
and the pass is renewed automatically.

| FIDÉLITÉ
ENROLMENT CANCELLATION… BY MEMBERS |
|
To cancel their enrolment in the “FIDÉLITÉ
Program”, members must complete the cancellation
form available on the STO’s Web site and submit
it to the STO’s Finance Department at least 10
business days before the next scheduled debit date1.
The Passe-Partout Plus pass will become invalid on the
first day of the month following the notice of cancellation.
If the cancellation request is submitted before the
one-year term has ended and for a reason other than
those accepted2, the member shall owe the discount of
10% for each month of use of the annual pass (every
month started shall be due). In addition, the Passe-Partout
Plus smart card linked to the enrolment must be returned
to the STO3 within 30 days, failing which an additional
amount of $20.00 (tax exempt) will be debited from the
account.

| FIDÉLITÉ
ENROLMENT CANCELLATION… BY THE STO |
|
The STO reserves the right to cancel enrolment
in the “FIDÉLITÉ Program”
in the following cases:
- If fraud
is established during processing of the file (false
statement, falsified documents, etc.)
- After the 3rd
debit rejection for a member in the last 12
months, the smart card linked to the debit payment
will become invalid immediately. Any bank rejection
fees (except for technical incidents beyond the
control of the payer) will be the responsibility
of the payer.
The STO reserves the right
to deny a member to enrol again if the past enrolment
was rightfully cancelled.

| FIDÉLITÉ
ENROLMENT MODIFICATION… BY THE STO |
|
The debit conditions for the “FIDÉLITÉ
Program” may be modified without advance notice
by the STO after a fare revision, since program members
agree to waive the pre-notification requirements of
the Canadian Payments Association of the amount(s) and
due date(s) debited from their account and every time
there is a change in the amount(s) or payment date(s)
of those debits. A public notice will be published in
the local newspapers when a fare revision occurs.

FIDÉLITÉ
PASS TYPE MODIFICATION… BY MEMBERS |
|
To modify the type of “FIDÉLITÉ
Program" pass, members must complete the
modification request form available on the STO’s
Web site and submit it to the STO’s Finance Department
at least 10 business days before the next scheduled
debit date1.

| CHANGE
IN FINANCIAL INSTITUTION |
|
The STO must be informed of any changes in
financial institution. Members must complete and sign
the modification form available on the STO's Web site
and submit it, along with a new sample cheque, it to
the STO’s Finance Department at least 10 business
days before the next scheduled debit date1. Members
must make the necessary arrangements to ensure there
is no interruption in monthly payments.

Members must inform the STO of their change
in address when they move by completing the modification
form available on the STO's Web site.

- Holders of an annual
pass agree to comply with STO regulations, in particular
the validation procedure when boarding buses.
- When boarding,
pass the Passe-Partout Plus smart card in front
of the reader to get the green light. Then show
the driver the side of the card with the photo.
- A lost or stolen
Passe-Partout Plus pass can be reissued with its
contents intact for a fee of $10.00 (tax exempt).
Defective cards will be replaced at no charge.

Members have certain rights of recourse if a debit is
not in accordance with this agreement. For example,
members have the right to receive a reimbursement for
any PAD that is not authorized or is not compatible
with this PAD Agreement. For more information on the
rights of recourse, members should contact their financial
institution or visit www.cdnpay.ca.

| STO
PASS ON THE OC TRANSPO SYSTEM |
|
Contrary to the monthly pass, the Passe-Partout
PLUS pass issued under the FIDÉLITÉ Program
does not require a proof of purchase (receipt) to use
the OC Transpo system. Apart from this difference, it
is subject to the same rules as other passes regarding
the supplements required on OC Transpo express routes.

annual enrolment can be cancelled at the member’s
request without any fees owing to the Société
de transport de l'Outaouais when the member is unable
to use the pass for a period less three months for
one of the following reasons:
- Death of the
member;
- Maternity or
parental leave, illness or accident – a certificate
must be provided;
- Unexpected move
away from the Outaouais area – proof of the
new address must be provided;
- Change in employment
– proof of the new place of employment must
be provided.
In all cases, the
STO's approval is required for the cancellation to become
effective.
3The “FIDÉLITÉ
Program” Passe-Partout Plus pass can be returned
by mail to the STO’s administration office at
111 Jean-Proulx, Gatineau, J8Z 1T4, or dropped off
at one of the STO service points (Galeries Aylmer,
Les Promenades or STO administration office).
