Updating Your Information on File

If you have been eligible for paratransit service for several years or you have been making reservations for someone else, the information in your file may no longer be up to date.

  • Have you moved?
  • Have your telephone numbers changed?
  • Have your emergency contact persons changed?
  • Has your medical condition changed?
  • Has the mobility aid (walker, cane, wheelchair, etc.) you use changed?

If you answered "yes" to any of theses questions or if you are not sure whether the information in your file is accurate, please take a moment to complete this file update form.

The Service Point can be accessed through the main door on the east side of the building.
