FIDÉLITÉ Program - Enrolment Procedure

Enrolment Procedure
   
1 Complete the form on-line
Make sure you complete all the mandatory fields. All the information is saved in a secure database that only the STO can access.


2 Print the form
Once you have completed the form, click "Validate and Print" to print the form.


3 Send the form
Send us the form with your cancelled sample cheque before the 15th day of the month preceding the month in which the pass is to start.

4 Pick up the new Passe-Partout Plus
You must then pick up your new Passe-Partout Plus at the STO Customer Service Point that you selected on your enrolment form. Your card will be ready to be picked up anytime during regular business hours, on the last week of the month.


5 Pre-authorized debit payments
The first debit is taken from your account on the 25th day of the month, preceding the month in which the pass is to start, or the next business day. All other monthly debit payments are taken on the same day of the month or the next business day.


6 Pass Renewal
Enrolment in the FIDÉLITÉ Program is annual and is renewed automatically until you request to cancel it. If you wish to cancel your pass, you must complete the cancellation form available on line at least 10 days before the next pre-authorized debit.
